Advancing Team-based Care webinar series
In 2016, the Community Health Center, Inc. (CHC) and its Weitzman Institute launched the National Clinical Workforce Development Initiative. As part of this work, CHC collaborated with the MacColl Center for Health Care Innovation to host a series of eight webinars on transforming health care practices through the team-based model of care.

Improving Cardiac Care through Health Coaching | Patient-Centered Primary Care Institute
Health coaching is a valuable mechanism for improving both cardiac care and self-management. Health coaching also helps patients gain the knowledge, skills, tools, and confidence to become active participants in their own care. Coaches help patients set their own health goals and then support their efforts to achieve these goals. This webinar presents five areas in which health coaching can provide assistance to patients.
