Healthy Hearts Northwest is a three-year project for primary care practices in Washington, Oregon, and Idaho that aims to help improve the cardiovascular health of patients by expanding on existing quality improvement capacity within small- to medium-size clinics. While recruitment into our project is now closed, practices are participating nationwide as part of the AHRQ EvidenceNOW initiative.

Prepare for new value-based payment models and maintain physician certification. Be ready for future payment systems that are based on value and quality measures and fulfill Part IV Maintenance of Certification for the American Board of Family Medicine and American Board of Internal Medicine.

Work with experts
Get support from experienced coaches who understand your practice and will facilitate quality improvement and provide guidance using your electronic health records. Join a learning community composed of other Northwest practices.
